JHA Definitions
When reviewing the job hazard form, you may find these definitions helpful --
Loss Potential Rating: The severity of the impact of the task to people or damage to the environment, equipment, property, and/or material before controls are put into place.
Major Hazard: Any activity or biological, physical or chemical hazard that has the potential to cause death, critical injury or lost time.
Moderate Hazard: Any activity or biological, physical or chemical hazard that has the potential to cause injury requiring medical services but not so great as to cause a lost time injury.
Minor Hazard: Any activity or biological, physical or chemical hazard that has the potential to cause injury requiring first aid or no treatment injuries.
Note: the main activities were rated for their loss potential before controls such as personal protective equipment or training are implemented.
Loss potential ratings can be altered to rate the task more hazardous but not less hazardous without approval from EOHSS.
